When you were admitted to the hospital, you were asked to list all of your medications, including over-the-counter drugs and vitamins and herbal supplements. Now that you are about to leave the hospital, you need to update the list and make sure you know how to manage your medications when you leave.   • Which of my original medications/supplements should I no longer take?
  • Will any medications I was prescribed in the hospital interfere with my original medications/supplements?
  • What side effects do I need to be aware of?
  • Who should I call if I have questions or problems with my medications?

How to Avoid Medication Errors

Be sure that all of your doctors know what medications you have been taking (they should have copies of your medication list). Be sure that all your doctors know about your allergies—to medications, anesthesia, foods, latex products, etc. When you are brought medications or IV fluids in the hospital, ask the person to verify that they are for you. Know Your Meds

While you are hospitalized, your doctor may prescribe medications for you. Use the Medication Checker to be sure you understand what your medicines are and why they are being prescribed.

  • What are the names of the medicines?
  • Why am I taking this medicine?
  • What dose will I be taking? How often and for how long?
  • What are the possible side effects?
  • Can I take this medicine while taking my other medications or dietary supplements?
  • Are there any foods, drinks or activities to avoid while taking this medicine?
